If you have one of the following conditions, you will be invited for an annual health check.

• Diabetes
• High blood pressure
• Kidney disease
• Heart disease
• Asthma
• Chronic bronchitis
• Rheumatoid arthritis
• Learning disabilities
• Dementia
• Severe mental health problems

We invite all patients in for their annual health check every year to make sure they are getting the best care possible. A patient may need a blood test before the appointment, in which case our reception team will arrange this with them.
