If you require any vaccinations relating to foreign travel, you are required to complete a travel risk assessment form.

The risk assessment form will ask you which countries and areas you are visiting, to determine which vaccinations you will need. We require the form to be completed and dropped off at Reception, a minimum of 6 weeks before your travel.

Click here to access the travel risk assessment form. Your completed form will be forwarded to our Nursing Team, who will assess your requirements and contact you by telephone to arrange any vaccinations that you may need.

Some vaccinations will require you to be referred to a travel clinic as either they are not available via the NHS or they are too complex. These will incur a charge which the travel clinic will discuss with you.
